Insurance Authorization Requirements
All patients should verify their health insurance coverage prior to their scheduled appointment.
Dr.Grabow's list of accepted insurances is posted on this site, due to plan variability please call our office to
confirm.

Co-payments
Co-payments are due at appointment check-in. Payments can be made by: Cash, Check, MasterCard or Visa.
Requests for Medical Records
To obtain a copy of your medical records, a completed release must be submitted. Click here.
To obtain your records from another physician's office please download and complete this form, then send it to the physician's office.
Request of Radiologic Studies
If you wish to obtain copies of your imaging studies, please contact the location where the studies were performed.
If you have left your studies at one of our offices and would like to obtain them, please call the office and arrange a time when they can be picked up.
